    Hi TJ Radio,

Song title (the TJ Radio song I played over):
My name or handle, as I want it credited:
Link to my lead track (WAV or MP3, just my instrument, no backing):
Where I come in (from the start, or e.g. at 1:42):
Where I'm from (optional):

I played this lead myself, I own it, and I give HMIC non-exclusive permission to stream the mix on TJ Radio and credit my handle. I'm 18 or older.
